*Smile Assurance is not insurance, but a discount plan. It cannot be used in combination with any other dental plan, workers' comp claims, referrals to specialists, insurance claims, or in conjunction with other discounts or promotions. Coverage is in effect for twelve months beginning on membership enrollment date with membership dues paid in full. Appointment times are limited. It is the responsibility of the member to schedule appropriate visits. Any dental procedure in progress or completed before or after a member's eligibility period is excluded.
